PROJECTSKINNYBOY RELEASE FORM: For the good and valuable consideration, receipt of which I hereby acknowledge, I hereby irrevocably and absolutely consent to the unrestricted use by projectskinnyboy and its heirs, assigns, and designees ("Photographer","Videographer","Sound Engineer"), and those acting with its permission authority to use any and all photographic or other images ("images") or video ("film") or sound ("audio") of me that projectskinnyboy creates/created or makes/made or receives/received of me or from me for all purposes, in any form, and in any and all media (whether now known or hereafter devised), including, without limitation, advertising, solicitation, stock photography, or trade, to copyright same in Photographer’s, Videographer's, own name or any other name in connection therewith if Photographer, Videographer so chooses. I waive any right to inspect or approve the finished images, film, advertising copy, text, or other printed matter that may be used in conjunction therewith, or to inspect or approve the eventual use(s) to which the images or film may be applied. I release and discharge Photographer, Videographer and those acting under its authority from any and all liabilities, claims and demands arising out of or relating to any blurring, distortion, or alternation whether intentional or otherwise, that may occur or be produced in connection with the images or film, or in connection with any processing, alternation, transmission, display, or publication of the images. This release constitutes the sole, complete and exclusive agreement between Photographer, Videographer and me regarding the images or film and I am not relying on any representation whether oral or written. I acknowledge that I have read and understand this release. If provided an email address, a copy of this release will be sent to me electronically.

I DECLINE to release any footage to PROJECTSKINNYBOY at this time.

General Release of Liability

You are not required to submit this section of our form, however not doing so will cancel your scheduled session. This section says basically you give up any and all rights to sue PSB should anyone become injured before, during or after a session, including yourself or anyone attending invited by you or invited by anyone you invited, allowed to attend.

For the good and valuable consideration, receipt of which I hereby acknowledge, I hereby irrevocably and absolutely release and forever discharge projectskinnyboy and its heirs, assigns, and designees, successors, personal representatives, affiliates, and any and all persons, firms or corporations liable or who might be claimed liable, whether or not herein named, none of whom admit any liability to the undersigned, but all expressly denying liability or harm, from any and all claims, demands, actions, causes of action or suits of any kind or nature whatsoever, which I now have or may hereafter have, arising out of or in any way relating to any and all injuries and damages or any and every kind, to both person and property, and also any and all injuries and damages that may develop in the future, as a result of or in any way relating to the following: PROJECTSKINNYBOY. It is understood and agreed that this Release contains the entire agreement between parties; and that the terms of this Agreement are contractual and not merely a recital. Furthermore, this Release shall be binding upon the undersigned and his representative heirs, executors, administrators, personal representatives, successors and assigns. This Release shall be subject and governed by the laws of the State of Massachusetts. I acknowledge that I have read and understood this release.

I DECLINE to release NIXON PHOTOGRAPHER of any liability.
